ACCESSIBILITY MODIFICATIONS IN PRIVATE HOMES AND FAMILY CARE HOMES AND IRAs
Developed and streamlined Architectural Barrier Consultation program to design and carry out home modifications for people with disabilities. Carried out interview/evaluations, design, and contracting phases of projects throughout the southeastern New York State region for Medicaid, Dept. of Health, and Community Based Services funded projects, as well as NY State Dept. of Education home modification projects. Work goals consisted of defining, documenting, planning and achieving home and bathroom access and other modifications for families having to care for family members with special needs, as well as researching and providing products directly related to the modifications. Also did workplace modification projects.
Home projects included written evaluation reports, plans and specifications for entire scope of projects and ongoing communications with families, counselors and care providers to tailor work plans for specific needs and abilities, locating contractors and obtaining bids for work, and providing job oversight until projects were complete. Included all internal and external agency paperwork and documentation, telephone contacts, scheduling, estimating, and payments to contractors.
Provided home modifications for consumers of Taconic DDSO, Hudson Valley DDSO, Sullivan County DDSO, VESID’s Middletown, Kingston and Poughkeepsie regional offices, Departments of Social Services in Rockland, Dutchess, Westchester, and Orange, and Ulster Counties, Orange County Adult Services, the Stony Ford Foundation, Sullivan Diagnostic, Rockland Commission for the Blind and Visually Handicapped, and Westchester Dept. of Mental Health.
